Per the Lincoln High School Handbook, "Any student participating in a sport at the High School must provide the school nurse-teacher with written evidence of a physical exam and clearance to play a sport on a yearly basis. If the clearance expires during the season, an update is required to complete the season.” Physicals are valid for one year. An up-to-date physical is required for participation in tryouts. This is a school department policy for the safety of the children. The Nurse’s Office is merely the record keeper and is not in the position to waive this requirement.
Please make sure a copy is on file with the school nurse. If you are unsure if the physical is on file, please call the Nurse’s office at 334-7522. The fax number is 334-8753. If you fax a physical to the Nurse, please call to ensure that it was received.
During the summer months, please place a copy of the physical in a sealed envelope marked “School Nurse”, with the name of the sport, and place it in the Nurse’s mail box in the Main Office. The deadline for submitting physicals to the Nurse for clearance is August 1, 2017.
